What does this test measure?

Here's the problem with standard estrogen tests: they lose accuracy at very low levels. And for men — or anyone with naturally low estradiol — that's exactly where you need precision. This test uses liquid chromatography with mass spectrometry (LC-MS), the gold standard for measuring hormone levels below 80 pmol/L. It catches what regular tests miss.

This test shines when you need to investigate puberty timing (early or late), track estrogen-producing tumours, evaluate amenorrhea, fine-tune hormones before IVF, or figure out gynecomastia in men. Men's estradiol lives in a range where standard tests just aren't reliable enough. This one is.

What a high value can mean

Elevated high-sensitivity estradiol in men may point to estrogen-producing tumours, liver disease affecting estrogen breakdown, or medication side effects. In women, it could indicate ovarian hyperstimulation, tumours, or excessive aromatase activity.

What a low value can mean

Very low estradiol may suggest hypogonadism, ovarian failure, hypothalamic amenorrhea, or the effects of aromatase inhibitor therapy. In men, low estradiol can quietly erode bone density and shift metabolic balance.

Associated symptoms

  • Gynecomastia in men
  • Delayed or absent puberty
  • Amenorrhea (absence of menstruation)
  • Bone density loss
  • Fertility problems
  • Changes in body composition

Who should consider this test

  • Men with suspected estrogen imbalances
  • Children or adolescents with puberty concerns
  • Women with amenorrhea or very low estrogen
  • Patients being evaluated for estrogen-producing tumours
  • Those in IVF requiring precise hormone tracking

Before your appointment

No special preparation needed. Bring valid ID. Note: this analysis is performed only by Karolinska University Laboratory, and sampling must take place at a Karolinska lab. If you take a supplement containing biotin (often sold for hair and nails), pause it for two days before the test, as high doses can distort the result.

  • Bring photo ID to the sampling location.
  • You do not need a doctor's referral — your referral is issued when you order.
  • Tell the staff which test you have ordered if they ask.

Your values appear in your account with reference ranges and an explanation of each marker.

Good to know

How does this differ from a standard estrogen test?

It uses mass spectrometry instead of immunoassay. Much more accurate at measuring very low estradiol (below 80 pmol/L) — which is exactly the range that matters for men.

Why is this test important for men?

Men naturally have low estradiol. Standard tests can return unreliable numbers in that range. This test gives you a precise reading where it counts.

Where can I take this test?

This test is only available at Karolinska University Laboratory sampling stations in Sweden.
